You must sign in before you can request a replacement card via the website. If you already have an account on, please sign in.
    As A Dive Professional, Dive Center click here to login with your existing account.

Sign Up for Your New Account

To obtain a replacement card, send or fax us a clear photocopy of your existing card (if possible), a clear photocopy of your training logbook pages, or a clear photocopy of your temporary certification card. Include these along with the Replacement C-Card Request Form (approx. 70 k). The form is in PDF format. You will need to download the Adobe Acrobat Reader (free) from Adobe to be able to read and print the form. If this does not work, you may obtain a form, by contacting NAUI Worldwide directly. We also need a copy of a photo ID (Driver's License, Passport, etc.) and payment. We accept money orders, Visa, MasterCard, and American Express. Replacement cards are $45 (US). Additional charges for rush shipment and other services may also be incurred. Please submit to us at:

    In summary, the steps are:

    1. Download the Replacement C-Card Request Form (approx 70k). Print this form and fill it out.

    2. Copy either your Temporary Card, Training Logbook Pages, your Original Card, or any other supporting documentation. Attach with the replacement request form.

    3. Copy a Valid photo ID (Passport, Driver's License, etc.) Attach with the replacement request form.

    4. Fax or mail the pages from the steps 1-3 above to NAUI Worldwide with payment. Only Credit Card orders are accepted via Fax. We do not accept orders via phone or email at this time.
